SGLT-2 Inhibitors Clinical Pharmacology & Therapeutic Promise in Veterinary Medicine

Presentation Description / Summary:

BACKGROUND Sodium-glucose co-transporter 2 (SGLT-2) inhibitors (e.g., dapagliflozin and empagliflozin) are effective antidiabetic therapies in human patients with type 2 diabetes mellitus (T2DM) and are associated with improved glycemic control as well as with reductions in body mass and blood pressure. In large cardiovascular outcome trials in patients with diabetes SGLT-2 inhibitors improve cardiovascular and renal outcomes, including hospitalization for heart failure, with this benefit extending to patients without T2DM who have heart failure with reduced ejection fraction.

Accumulating data from several large, placebo‐controlled trials, suggest that SGLT‐2 inhibitors may be beneficial in the treatment of cardiovascular diseases regardless of the patient’s diabetes status. Besides their effects on glucose excretion, SGLT2-inhibitors also positively impact systemic metabolism by reducing inflammation and oxidative stress, shifting metabolism towards ketone bodies production and suppressing glycation end-product signaling.

RATIONALE Because of their effect on cell metabolism, SGLT-2 inhibitors could be beneficial in a wide array of canine diseases, including but not limited to, congestive heart failure, hypertension, obesity and chronic kidney disease. This multi-systemic effect provides an opportunity to target several therapeutic indications using a preclinical model that recapitulates features of metabolic dysfunction (MetD). This lecture series will showcase the value of combining basic and comparative pharmacology knowledge with clinical medicine for the development of a potential new breakthrough therapeutic in veterinary medicine.

Learner Outcomes: Through this lecture series, attendees will learn:

• The basic and clinical pharmacology of SGLT-2 inhibitors.
• The metabolic variations that accompany cardiorenal diseases and T2DM and the relevance of SGLT-2 inhibition therapy in these indications.
• How information from human clinical trials can inform the development of SGLT-2 inhibitors in veterinary medicine.
• How preclinical pharmacodynamic information in dogs can inform the molecular basis for the efficacy of SGLT-2 inhibitors in heart failure patients without T2DM.

Learning Objectives:

  • Upon completion, participants will have a good understanding of the systemic effects of westernized diets in dogs and how these could promote disease development long-term.
  • Upon completion, participants will have a broad overview of the therapeutic effects of sodium-glucose transporter inhibiteurs and how they can be beneficial in the context of cardiovascular diseases.
  • Upon completion, participants will understand how data generated in human clinical trials can inform clinical development of SGLT2 inhibitors in veterinary medicine, and vice versa.
